Use our form to notify us about a structure rehabilitation where the structure has been reopened or will soon reopen to traffic. Do not submit a form until a reopening date has been set. Submit this form as soon as possible. Projects that qualify as major rehabs require the bridge to be inspected within three months of reopening to traffic.
